Personnel (CAD PERS) Update Form (Law or Fire)

Use this form to change personnel information for CAD, such as assignments, promotions, changes in pager/telephone numbers, etc.

Premise Hazard Form (Law or Fire)

Use this form to report responder safety information to be entered into the CAD Premise/Hazard file.  This may include hazardous materials on scene, drugs, weapons, known threats to law or fire personnel, and other similar hazards.
